Must be a public entity able to enter into an intergovernmental agreement (IGA) with the MWRD. Eligible public entities include municipalities, townships, and other local government organizations.
Administrative Head or Executive of the Applicant
Individual that will sign and submit the final, full application to the MWRD. Examples include Public Works Director, Engineering Director or an equivalent position.
Application Preparer or Facilitator
Individual knowledgeable about the details of this application, authorized to prepare and review applications on behalf of the Applicant.

List each property proposed for voluntary acquisition.
Provide the full address (street city, state, zip), fair market value of the property, and the 14-digit PIN(s) associated with the structure and property.
Property to be acquired must located within MWRD's Corporate Boundaries. Property must have structures that are at-risk of being damaged or destroyed due to flooding. These structures must be located within either the FEMA's Special Flood Hazard Area or MWRD's Detailed Watershed Plan Inundation Area. Contact MWRD if more than 20 properties are to be submitted.
Briefly categorize the current uses of the listed properties (For example: residential, commercial, apartments, vacant lots, etc.)
What are the current site conditions of the properties listed above and the surrounding area?
Excluding weather conditions, describe how the listed properties are currently used, the properties in the surrounding area, nearby bodies of water, unique geographic features, any historical associations, known environmental factors, and any other relevant details.
Describe the type and impact of flooding that occurs within and around the properties listed. What is the depth of flooding for the impacted properties and streets?
How has your public entity (Applicant) intervened due to this flooding? Have there been any conversations with the owners of the listed properties regarding a voluntary, flood-prone property acquisition offer.
Provide any additional site condition details, expanding on the three questions above if needed. Please reference specific properties where needed.
What is the long-term use of the properties once they are acquired? What is the proposed site restoration once the properties are acquired?
How will this project affect your maintenance operations?
Describe any additional maintenance tasks needed and any reductions in work due to the project.
Summarize the project's capital costs below.
Estimate the costs to acquire, demo, and convert the proposed properties to open-space. Under this program, MWRD funding can only be used to reimburse the purchase price of the flood-prone properties.
Summarize the project's financing sources below as well as the status of that funding.
Include the requested MWRD contribution, the Applicant's contribution, and contributions from federal, state, and local sources.
